ENROLLMENT VERIFICATION FORM (To be submitted by selected scholars AFTER the fall term has begun)
The Enrollment Verification Form is required ONLY from students who have been selected to receive an award. This form is to be completed by your school's Registrar office and must be officiated with the school stamp/seal. This form will only be accepted once you have begun your classes at your named college or university.

FREE APPLICATION FOR FEDERAL STUDENT AID (FAFSA)
The FAFSA is made available free of charge by the U.S. Department of Education for the purpose of applying for federal and state student grants, work-study, and loans. HSF requires all scholarship applicants to apply for federal financial aid. To obtain a FAFSA, please visit www.fafsa.ed.gov.
GPA CERTIFICATION FORM
The GPA Certification Form is required from students whose transcripts do not report or display a GPA on a 4.00 scale. Your academic counselor, registrar or other school official must sign this form, verifying your GPA on a 4.00 scale. (To download, click on Word Document or PDF Document)

STUDENT AID REPORT (SAR) -- (Sample 1)
A Student Aid Report (SAR) is required as verification that you have applied for federal financial aid. You will receive a SAR from the U.S. Department of Education after you have filed the Free Application for Federal Student Aid (FAFSA). The SAR must include your name, SSN, citizenship status, and Expected Family Contribution (EFC) all on the same page.
